Well, i guess "That One Isekai"  is what you look for. It's absolutely demented, and kicks each single isekai anime trope in the face, exaggerating it to  hilarious levels. And the anatomy... the anatomy descriptions are eldritch. 

Unsong - by far the most ridiculous web fiction I've read.  It's not bad, just insane.  It would maybe best be described as "Kabbalistic apocalypse in the modern day"?

Dao of Magic - cultivator Ascends, but finds himself in a shithole universe.
