I think a fourth key ingredient ought to be added to your list.  And that is:


4) Exhaust all other possible outcomes.

As an author you should give your Hero every chance to save his life. Give him every option. None of us are willing to throw our lives away willy nilly, and a true hero makes sure that, if death is inevitable, they have gone out fighting all the way, both with their brains and with their brawn.

I like that... and you may have saved at least one character's life by adding that point.
